Primary Focus:

The Non-Clinical Care Coordinator provides Case Management Services to support the Oregon Army National Guard (ORARNG) Soldier's health and human service needs. The Non-Clinical Care Coordinator shall be familiar with the regulations and policies that apply to Individual Medical Readiness (IMR) and deployment. The position is to assist the ORARNG in adhering to the Military Health System's (MHS) three primary goals for Case Management (CM): Improving the care, management and transition of recovering service members; Broaden the application of CM to include those with complex and at-risk needs before the Soldier requires complex care; Evaluate the impact of CM on the quality of military health care.

Responsibilities

  • Manage the care and associated documentation of each assigned Soldier in accordance with the individualized treatment plan.
  • Tracking medical status of ALL assigned Soldiers.
  • Medical charting in eCase (MEDCHART).
  • Request profile exams/evaluations and audiology exams at physical exam center NLT than 7 business days of interaction with service member.
  • Coordinate Soldier treatment plans with appropriate medical facilities NLT 10 business days following medical appointment.
  • Maintain 95% of all automated referrals indicated in MEDCHART.
  • Submit case processing, medical documentation, and profile concerns to military/contacted provider.
  • Attend Medical Readiness events (also known as PHAs) events as determined by the needs of the state and the individual units. At minimum one event will be attended each year.
  • Ensure Soldiers referred by a provider are followed by the case management team within 7 calendar days of referral.
  • Manage Soldier’s care from the identification of the condition to when the Soldier is either considered medically ready or is administratively separated from the military.
  • Follow the regulations and policies applicable to Individual Medical Readiness (IMR) and deployment, to include the AR 40-501 and other applicable guidance.
  • Ensuring all, with <5% margin or effort, medical documentation received from the Soldier and/or their medical team is received and placed in appropriated electronic record, as well as the Soldier’s hard copy record.
  • Maintain all Soldier medical records in accordance with Army Regulation (AR) 40-66.
  • Provide records custodian or delegate with all relevant medical/dental documentation.
  • Obtain release of information documents from Soldier within 15 calendar days of interaction with service member.
  • Educate all Soldiers on resources available to improve medical and dental readiness.
  • Maintain Health Insurance Portability and Accountability Act of 1996 (HIPAA) standards and guidelines.
  • Review and track all, with a <5% margin of error, (PHA, SRP) lab results contact SM and educate on lifestyle modifications.
  • Upload, stamp and index all, with a <5% margin of error, medical documents of case managed Soldiers into Health Readiness Record (HRR).
  • MODS entry of immunizations, designation of pregnancy, eye exams, medical warning tags of all soldiers, with a <5% margin of error.
  • File all medical documents received for case management to Records Custodian using a document evidencing transfer of custody from the Contractor to the government. Copy of each supporting transfer document shall be provided to the COR at the end of each month, submitted no later than 7 calendar days through the beginning of the next month.
  • Track all medical appointments by communication with unit to ensure Soldier attendance and submission of follow up documentation.
  • Log in eCase any contact with SM (email, voice call, or personal interview). Contractor will ensure all administrative charting in eCase is completed with a <5% margin of error.
  • Communicate with delegated government personnel to track compliance versus noncompliance at minimum once a quarter with an expected 100% compliance rate.
  • Run and monitor MEDPROS all, with a <5% margin of error, reports Post PHA/SRP events to ensure all scheduled services are completed/updated and current medical status is reflected.
  • Report Medical Readiness to the Commander or his/her designated Representative within 30 days of determination.
  • Ensure all, with a <5% margin of error, Soldiers with T3, T4, P3 or P4 profiles are case managed in the eCase module.
  • Must be able to recognize and respond appropriately to urgent/emergent situations per protocols.
  • Maintain work area, exam rooms and supplies.
  • Maintain professional appearance and personal conduct at all times.
  • Attends meetings as requested.
  • Follow all ORARNG policies and procedures.
  • Adhere to employer work practices as described in Cayuse policies.
  • Become familiar with and obey regulations on the installation, including fire, traffic, safety, and security regulations.
  • Perform additional reports as required.
  • Other duties as assigned.

Qualifications

Minimum Qualifications:

  • Minimum education requirements: high school diploma or GED
  • Familiarity with medical terminology and medical documentation
  • Experience with patient care coordination
  • Preferred experience:
    • Medical Data Protection System (MEDPROS)
    • eCase (MEDCHART)
    • Health Readiness Record (HRR)
  • Be experienced in all aspects and qualified in respective functional areas. Contractor personnel shall not perform inherently governmental functions (i.e., representation of government, decision authority, etc.).
  • Read, understand, write, and speak fluent English.
  • Must be able to pass a background check. May require additional background checks as required by projects and/or clients at any time during employment.

Minimum Skills:

  • Highly proficient in Microsoft Office Suite.
  • Must be able to communicate clearly to patients in person and over the telephone.
  • Visual acuity adequate to perform job duties, including visual examination of patient and reading information from printed sources and computer screens.
  • Adequate physical ability including sufficient manual dexterity to perform the requisite health care service. Requires frequent bending, reaching, repetitive hand movements, standing, walking, squatting and sitting, with some heavy lifting, pushing and pulling exerted regularly throughout a regular work shift.
  • Must possess problem-solving skills.

Reports to: Cayuse Health Program Manager

 

Working Conditions:

  • Professional office environment.
  • Telework may be authorized as required by projects and/or clients at any time during employment.
  • Keep regular clinic hours as approved by Manager/Director - 7:00am - 4:30pm.
  • Must be physically and mentally able to perform duties extended periods of time.
  • Ability to use a computer and other office productivity tools with sufficient speed to meet the demands of this position.
  • Must be able to establish a productive and professional workspace.
  • May be asked to work a flexible schedule which may include holidays.
  • May be asked to travel for business or professional development purposes.
  • May be asked to work hours outside of normal business hours.

Pay Range

USD $20.00 - USD $23.99 /Hr.
