Qualifications – Here’s What You Need:
- Master’s degree in social work from a school accredited by the Council on Social Work Education.
- Must possess a current State of D.C. license to practice as an Independent Practitioner with eligibility and willingness to maintain good standing for the duration of employment.
- Only Clinical Social Worker, Independent Clinical Social Worker or Certified Social Worker licenses are qualifying for this position.
- Ph.D is strongly preferred but not required.
- One year experience with providing admission preparation, assessment, counseling, day-to-day assistance, initiation and revision of care plans and integrated services to effectively coordinate resident/patient care within a geriatric population
- Experience with long-term care and interacting with geriatric patients is highly preferred.
- Substantial experience in healthcare settings dealing with geriatric population, dementia, and/or medical and psychiatric social work skills.
- Ability to successfully pass a minimum Tier One background check, including initial and any subsequent or periodic reinvestigations as may be required during employment, and to meet all eligibility requirements for access to secure federal facilities and systems.
Minimum Skills:
- Ability to read, write, and speak English proficiently to effectively communicate with employees, providers, and other stakeholders, and to accurately document clinical encounters and health information.
- Clear and professional communication skills in both face-to-face and telephone interactions, with the ability to convey information in an understandable manner.
- Exceptional oral and written communication skills, including the ability to prepare concise documentation, reports, and correspondence.
- Strong customer service orientation, with the ability to respond promptly, professionally, and with an appropriate sense of urgency.
- Proficiency with Microsoft Office (e.g., Word, Excel, Outlook, PowerPoint) and Adobe toolsets, including the ability to create, edit, and manage documents, spreadsheets, and forms.
- Demonstrated ability to manage multiple tasks and priorities simultaneously in a fast-paced environment while maintaining accuracy and attention to detail.
- Self-starter with the ability to work independently with minimal supervision, as well as collaboratively as part of a multidisciplinary team.
- Sufficient visual acuity to perform all essential job functions.
- Adequate physical ability and manual dexterity to perform duties, including frequent bending, reaching, repetitive hand movements, standing, walking, squatting, and sitting, as well as regular lifting, pushing, and pulling as required.
- Strong organizational skills and effective time management, with the ability to prioritize tasks and meet deadlines.
- High level of self-motivation and the ability to quickly learn and apply new concepts, clinical practices, software systems, and technical skills.
- Demonstrated problem-solving skills, including the ability to assess situations, identify issues, develop options, and implement appropriate solutions within scope of practice and established protocols.
Reports to: Program Manager
Working Conditions:
- Professional office setting.
- As an essential position critical to AFRH operations, reporting to work on time as scheduled is required, even during emergency situations or severe weather events.
- Must be able to establish and maintain a productive, organized, and professional workspace.
- Professional attire is required at all times.
- Complete all AFRH training within 30 calendar days of reporting and annually thereafter.
- Complete all Cayuse training within 15 days of reporting and annually thereafter.
